What is a dental implant and how does it work?

A dental implant (dentures) is like a little anchor made of titanium. It replaces the root, that has been removed, in the jaw. After the implant has firmly grown together with the jaw, the dentures itself can be fixed on it. The aim is to achieve a function and look that is as similar to that of a natural tooth as possible. Dental implants can be applied in different situations: for the replacement of existing single teeth as well as for larger gaps, and they serve as anchor points for total dentures.

What are the advantages of dental implants?

Centerpulse implants offer you a variety of advantages:

Healthier and more beautiful
When teeth are missing, the surrounding bone will develop back. This unfavourable loss of bone material leads to a reduced alveolar ridge; this can also lead to a retreat of the gum which is optically negative. Implants can help to counteract this problem so your face can keep its natural profile. As an alternative to traditional bridges that require the polishing even of intact neighbouring teeth, implants can be used in a way that the whole tooth substance can be maintained.

More comfortable
Because implants are anchored safely and firmly, your dentures will not loosen or move. With implants the greatest concerns of denture wearers, namely a bad sit, gum problems, and painful pinching, will not occur. Implant-assisted dentures can do without the otherwise annoying brackets or palate plates that might cause a retch stimulus. Especially your sense of taste will profit from it.

Are you an implant patient?
If you are healthy enough to have a tooth extracted, then you are probably also heakthy enough to get an implant. A good general health and a sufficient bone strukture of the jaw are the basic conditions. Your doctor will tell you whether an implantation will be an option in your case.

Which steps does an implantation comprise?
An implantation treatment comprises several treatment steps that span over a period of 2 - 9 months. The main steps are:

The implantation
The dentist inserts a Centerpulse implant into your jaw that is adjusted individually. In the following months the bone will grow firmly around the implant so it will be anchored in the jaw. During this healing period you will wear a provisional denture.

The dentures
In this step the dentist fixes a "post" on the implant. He takes an impression by the help of which the natural, functional dentures will be manufactured individually in the laboratory in the following days.

A) 4 implants have been inserted in the toothless lower jaw to stabilize the dentures.
B) Complete dentures in the upper jaw, fixed directly on the implants.
C) A single denture in the upper jaw, connected firmly to the implant.

Dentures

The term "third teeth" is often, wrongly, used for removable part and full dentures. Actually third teeth are any kind of dentures, that is: inlays, crowns, implants, part and full dentures. If healthy teeth still exist, the dentures can be fixed onto them. In case of no teeth left, the dentist will insert full dentures that adhere onto the jaw. Bridges, part and full dentures are suited for patients who dislike implants or cannot get implants.

Crowns

If there is still a part of a defective tooth left, a crown will be put over the rest of the tooth. There are all-metal crowns - e-g- made of gold - metal-free ceramic crowns, and blinded metal crowns with a core of casted metal that is covered with tooth-coloured porcelain.

Bridges

If more than three teeth are missing, the patient needs part dentures. It can be removable or fixed, and it can be made of metal, ceramic, or plastics. The substitute teeth are placed on a metall or plastic frame. There is also the possibility to connect still existing "pillar" teeth by a bridge on which removable dentures are attached with push buttons, connectors, or bolts.

Teilprothesen

Fehlen mehr als drei Zähne, benötigt der Patient eine Teilprothese. Sie kann abnehmbar oder festsitzend sein und aus Metall, Keramik oder Kunststoff bestehen. Die Ersatzzähne sitzen auf einem Metall- oder Kunststoffgerüst. Es besteht auch die Möglichkeit, noch vorhandene Pfeilerzähne mit einem Steg zu verbinden, an dem eine herausnehmbare Prothese mit Druckknöpfen, Steckverbindungen oder Riegeln angebracht wird.

Full dentures

If there are no teeth left at all, full dentures are used. They are made of dentures plastics - either with or without a metal basis - or of soft special plastics, especially for the lower jaw. In the case of removable dentures the problem occurs, though, that the toothless jaw will recede over time and the dentures then have to be padded.

AESTHETIC DENTISTRY

Brightening of teeth

The dentist will examine the teeth at the start of the treatment thoroughly. He will determine of what nature the discolourations are and what caused them. Before the brightening itself, a caries or root treatment may be necessary. Often enough a clear "beautification" of the teeth can be witnessed already after such a measure. But now it can also be clearly seen which discolourations are situated inside a tooth and therefore cannot be treated solely with a superficial cleaning.

The further procedure then depends on whether

  • the whole set of teeth has to be brightened, or whether
  • only single teeth are affected and darker than their neighbours, and therfore should be treated.
If the whole set of teeth has to be brightened, the dentist will take an impression of it. This then will be used to manufacture a plaster model on which a thin, soft plastic splint will be placed, individually for you. This splint will cover only the set of teeth, not the gum.
The dentist will adjust it exactly inside the mouth. In most cases you will carry this splint back home to wear it during the following nights; therefore the dentist or his assistant will show you how you should fill in the Opalescence PF-gel before going to bed. You may also only wear the splint a few hours at daytime; in this case the dentist can give you a higher dose of the Opalescence PF-gel which you should let soak in between two to four hours.
Opalescence is available with different tastes: neutral, mint, and melon. At home you just go on brushing your teeth as you are used to and then put the splint over your set of teeth. After the soaking in you remove the splint again, brush your teeth and also clean the splint thoroughly. The dentist will arrange with you control examinations in the practice to observe the progress of the brightening. Under certain conditions the treatment with the splint can also take place directly at the dentist; then the splint - covered with Opalescence quick-gel - is inserted in the practice and remains in your mouth for about one hour before it is removed again.
In most cases 4 to 10 treatments are needed for the brightening. If single dark teeth have to be brightened, this can be done - apart from the method with the splint described above - quite quickly directly on the patient's chair in the dentist practice. For this purpose the affected tooth will be covered with Opalescence Xtra Boost-gel that has to soak in for about 15 minutes. Devital teeth (i.e. "dead" teeth with no dental pulp left) can be treated with this method from outside and from inside at the same time which makes this procedure more effective.

In most cases one session in the practice is sufficient when being treated with Opalescence Xtra Boost. With the help of Opalescence Xtra Boost of course also the whole set of teeth can be brightened but this would make a longer treatment time on the patient's seat necessary.


Yet another method must be mentioned: A single, devital ("dead") tooth can also be brightened by means of an Opalescence Endo inlay. This inlay will remain for 1-2 days inside the tooth and will afterwards be removed or exchanged by the dentist. You should observe your dark tooth carefully while it has the inlay. As soon as it becomes as bright as the neighbouring teeth, you should immediately consult your dentist to prevent the tooth from "over-brightening".

A combination of these brightening methods is possible. The dentist may choose the appropiate procedure from the Opalescence system according to your individual situation.
